1) We should say "May God bless you" and we omit the first word because we take it for granted.
2) They mean two different things. As was stated, "God bless you" implies "May God bless you" which is wishing or praying for a blessing from God. "God blesses you," on the other hand, means a knowledge or assumption that God definitely is already blessing you.
